A reassuring continuum of care
Our principal goal at Glenmeadow is to provide you with peace of mind about tomorrow. Offering you multiple service options and a continuum of care is essential in addressing concerns about your future, even as you stay active and independent. Residents have access to professional, full-time social services staff and licensed nursing staff 24 hours a day, seven days a week.
Our director of resident services, a licensed social worker, can schedule medical appointments, coordinate support through Glenmeadow at Home or the visiting nurses, help with insurance paperwork, complete healthcare proxies, work with hospital and rehab discharge planners on your behalf, and assist with a variety of other requests.
Companion services and personal care
Residents in our independent living apartments sometimes need a helping hand with daily tasks. Glenmeadow at Home is here to help with errands, laundry, or even bathing and dressing. Aides provide assistance and companionship for as little as an hour, or up to 24 hours. Residents in assisted living apartments receive companion services and personal care as part of their care plan.
Our registered dietitian can review and discuss your nutritional needs, and then help you determine the best way to nurture your dietary health.
For independent living residents, medication management includes regular contact with your physician regarding your medications, adjustments, and refills. Add-on services can include ordering medications, setting up a weekly pillbox, and administering medications at the nurses’ station or your apartment. For assisted living residents, all of these services are included in the monthly fee.
Nursing services and emergency response
Glenmeadow is staffed with licensed nurses on-site 24 hours a day to respond to emergencies, coordinate medication management, perform blood pressure checks, and a host of other services. Our wellness nurse, dedicated exclusively to independent living, takes a proactive approach that focuses on prevention and healthy living. Independent living residents receive 40 nursing visits per year as part of their monthly fee, and can arrange for additional visits and services for a fee. Assisted living residents receive nursing visits at no charge as outlined in their individual care plan.
Transportation to medical appointments
Glenmeadow provides transportation to medical appointments within a seven-mile radius at no additional charge. Service is available several days and times each week, making it easy to fit your schedule.
